Transition Coordination Services for Young Adults

Northeast Career Planning's Transition Coordination Services ensures that students are prepared for employment, post-secondary education and/or community living when they enter the adult world.

The transition coordination process at Northeast Career Planning addresses a wide array of young adult skills for learning, working, community living and self-determination. In addition to what the students
learn while in school, NCP assists in the planning for what types of services and goals a young adult will need once they leave school. Through NCP's comprehensive transition process, students and their families are equipped to make informed life decisions as they continue the transition in the world of work.

Transition Coordination Services include:

  • Vocational consultation during the development of the students Individual Education Plans (IEP's)
  • Technical assistance to ensure effective implementation of the service components to the IEP
  • Inservice training is available for school staff, students and their families relating to all aspects of transition planning
  • Assistance with linkage and referrals to community resources
  • Linkage to post secondary education and vocational training support services
  • Travel training and/or assistance with transportation
  • Assistance with referrals to adult supports including ACCES-VR (formerly VESID), Office for People with Developmental Disabilities (OPWDD) and Office of Mental Health (OMH) which provide funding for a variety of programs and services.
  • Provide information and support to families throughout the transition.
